Inclusion
Hackney residents over the age of 18 with a suspected or diagnosed learning disability.
This will be as a result of their assessed IQ and adaptive functioning levels.
The Integrated Learning Disabilities Service will assess whether an adult has a learning disability and what support they are entitled to, based on an assessment against the eligibility criteria.

How to refer
Who can refer: GP, Nurse, Other, GPs, statutory and voluntary organisations, family and carers, Self-referral available
Document referral
Send by: email — <p>Referrals are accepted from GPs, statutory and voluntary organisations, family and carers but the person being referred must be aware of the referral.</p> <p>Referrals should be made on a joint referral form</p>
Self-referrals are also permitted. Referrals should be made on a joint referral form.
