Neighbourhoods & Networks
City & Hackney Integrated Primary Care (IPC) connects all 37 GP practices in City & Hackney through eight Primary Care Networks (PCNs) and local neighbourhood teams. Together, we make sure healthcare is joined up, community-based, and focused on the people who live here.
What are neighbourhoods?
Neighbourhoods are the foundation of how health and care are delivered locally. Each neighbourhood brings together GP practices, community health services, social care, and voluntary organisations to provide more coordinated support for residents. Neighbourhoods are designed around natural communities of 30,000 to 50,000 people. This makes it easier for teams to work closely with patients, families, and community partners. Each neighbourhood team includes professionals such as GPs, nurses, pharmacists, care coordinators, physiotherapists, and social prescribers, all working together to meet local needs.

What are Primary Care Networks (PCNs)?
Primary Care Networks are groups of GP practices that collaborate to improve services and share resources. Every practice in City & Hackney is part of one of the eight PCNs that make up City & Hackney IPC.
Each PCN has a Clinical Director and shared staff who provide care across practices. This includes pharmacists, physician associates, paramedics, health and wellbeing coaches, and social prescribers. Working at this scale helps deliver new NHS services, improve access to care, and support patients with long-term conditions.

Each network supports the GP practices and patients within its local area. Together they cover the entire borough of City & Hackney and the City of London.
