City & Hackney Integrated Primary Care (IPC) leads and supports public health campaigns that help people in City & Hackney stay healthy, prevent illness, and access the care they need. We work with Primary Care Networks (PCNs) and NHS North East London to share trusted information, reach every neighbourhood, and reduce health inequalities.
Our campaigns focus on prevention, early diagnosis, and everyday wellbeing, helping residents make informed choices about their health.
Stop Smoking Service
Gloji | Smokefree City & Hackney supports adults and children aged 12 and over who live, work, study, or have a GP in Hackney or the City of London. It provides 12 weeks of support with a dedicated advisor and access to stop smoking medication. Support is available either over the phone or in person.

Phlebotomy (Blood Tests)
Your GP practice provides blood tests, so you do not need to travel to the hospital. Tests are carried out by experienced nurses and healthcare assistants in a familiar setting, saving you time.

Wound Care
Following surgery, you can have your wound dressing changed and stitches removed at your GP practice on any day of the week rather than having to go to the hospital.

NHS Health Checks
5-yearly health check available to patients aged 40 – 74 who do not have a pre-existing health condition, covering blood pressure, weight, diet, exercise, alcohol consumption, and smoking status to assess your future risk of heart disease or stroke so you can make any necessary lifestyle changes. Depending on your risk, you may have a blood test. Routine blood tests include cholesterol, diabetes risk, and HIV screening.
Early Years Service
Pregnant women from early pregnancy through to postnatal care.
New mothers – helping you adjust and stay healthy after your baby arrives.
Women with long-term health conditions who are planning on getting pregnant – ensuring you receive the right care and advice before and during pregnancy.
Vulnerable children - the GP and health visitors work closely to coordinate care

Proactive Care: Home Visiting And Practice-Based Services
If you are housebound, very frail and have complex health needs, your practice GP, nurse or pharmacist can spend more time with you, either by visiting you at home, or seeing you at the practice. This includes creating a personalised Care Plan using the Universal Care Plan platform (UCP) to help support you to maintain good health and wellbeing.

There are 9 GP practices that provide specialist warfarin monitoring to patients who are stable on their medication, so that these patients do not need to go to the hospital to be monitored. As this service is not offered by all City and Hackney GP practices, warfarin patients may therefore be monitored by a GP HUB centre.
Mental Health
If you are experiencing mental health challenges and are currently taking medication, you can book an appointment with a GP or nurse for a comprehensive review. This is an opportunity to discuss both your mental and physical health and explore any additional support that may be beneficial to you.
Sexual Health Services
Your GP practice provides confidential, non-judgemental sexual health advice and care for people of all genders, ages, abilities, and sexual orientations. We can support you with contraception, pregnancy planning, STI testing and treatment, HIV prevention, and concerns about sexual wellbeing or pleasure.
