Primary Care Networks
City & Hackney Integrated Primary Care (IPC) supports eight Primary Care Networks. These networks bring together all 37 GP practices to improve access, coordinate services, and provide care that is closer to home. This page explains how PCNs work and presents the programmes delivered within each network.
How Primary Care Networks Support Local Residents
Primary Care Networks help practices work together so patients can access a wider range of services. In practice, this means shared teams of pharmacists, physiotherapists, social prescribers, care coordinators, and health coaches. PCNs focus on prevention, support people with long term conditions, and link patients with services across their neighbourhood.

Social Prescribing & Wellbeing Link Workers
Delivered through our partner the Family Action, this service supports adults (18+) registered with a GP in City & Hackney who are socially isolated, living with a long-term condition, or presenting with social issues such as financial hardship or mild-to-moderate mental health concerns. Patients are offered a personalised needs assessment and then connected with local community activities, peer groups, benefits advice, volunteering, physical activity or creative workshops — all with the aim of improving holistic wellbeing.
Cancer-Specific Social Prescribing
Through the Macmillan-funded extension of our social prescribing pathway, Family Action provides support to people aged 18+ in City & Hackney who have received a cancer diagnosis. The service includes a health needs assessment tool, followed by co-creation of a plan linking the person into community-based services, groups and activities tailored to their interests, helping them to live well during and after treatment.
Community Health & Wellbeing Programmes
Commissioned via Shoreditch Trust, this strand of service offers flexible, creative and practical programmes for residents to enhance emotional, physical and social wellbeing. Activities include health and wellbeing coaching, creative arts groups, walking and social clubs, cooking and lifestyle workshops — all designed to build personal resilience, enlarge support networks, and reduce dependence on statutory services.
Mental Health & Daily Living Support Network
In partnership with East London NHS Foundation Trust and its voluntary sector network, this service supports adults living with severe and enduring mental health conditions in City & Hackney. It offers tailored support focussed on mental wellbeing, physical health, social networks and daily living skills — delivered through a “Any Door” referral model enabling consistent access to the network.
