At City & Hackney Integrated Primary Care CIC, we team up with partners to help make primary care better and support the health of local residents. We help everyone work together – from GP practices to the NHS, local councils, and community groups.

We want to make it simple for partners to connect with primary care on a bigger scale, but we also make sure everything still fits with what local neighbourhoods actually need.

How We Work With Partners

  • We help coordinate some primary care services for all the GP practices in City & Hackney.
  • We support the design and running of local and neighbourhood services.
  • We’re a delivery partner for primary care programmes that have been commissioned.
  • We offer a single point of contact for anyone wanting to connect with general practice at scale.
  • We help with workforce, digital, and day-to-day operations across practices.
  • We work closely with everyone in the system to make sure services match what’s needed locally, help tackle health inequalities, and support joined-up care.

Our job is to connect the dots between big-picture strategy and real-life delivery, making sure plans actually turn into practical, sustainable support for primary care.

Commissioning and Service Delivery

IPC helps deliver and support a variety of commissioned services for our partners, including programmes that cover the whole borough and ones that focus on specific neighbourhoods. We bring together clinical leaders, people who know how to run things, and local know-how so services get delivered efficiently and smoothly.

We also help run pilots, transformation programmes, and new ways of delivering care, working with partners from the first idea right through to making it happen.
